Skin Permeation Studies and Approaches on Design of Manufacturing Processes. Abstract. This chapter deals with the use of in vitro skin permeation experiments to support industrial drug development and gives advice on how to implement process development steps for transdermal matrix patches. Using two different examples, the relevance of in vitro skin permeation studies as a useful tool during product development is discussed. These studies are considered of high value in establishing a ranking of various formulation concepts and for direct comparison between a test and a reference product and show that skin permeation data need to be evaluated with caution, keeping the different conditions between in vitro and in vivo settings in mind. Moreover, the importance of manufacturing process development and how critical process parameters can be linked with critical quality attributes are presented. A deep understanding of the manufacturing process should serve as basis for a robust and stable process design capable of producing a drug product of a defined quality. Keywords. A newborn infant sleeping in an isolette. A neonatal intensive care unit (NICU), also known as an intensive care nursery (ICN), is an intensive care unit specializing in the care of ill or premature newborn infants. The first American newborn intensive care unit, designed by Louis Gluck, was opened in October 1. Yale–New Haven Hospital.[1]NICU is typically directed by one or more neonatologists and staffed by nurses,[2]nurse practitioners, pharmacists, physician assistants, resident physicians, respiratory therapists, and dietitians. Infant respiratory distress syndrome (IRDS), also called neonatal respiratory distress syndrome, respiratory distress syndrome of newborn, or increasingly surfactant. · As part of a supplement entitled "Born Too Soon", this paper focuses on care of the preterm newborn. An estimated 15 million babies are born preterm, and.
Many other ancillary disciplines and specialists are available at larger units. The term neonatal comes from neo, "new", and natal, "pertaining to birth or origin".[3]Neonatal nurse practitioners are advanced practice nurses that care for premature babies and sick newborns in intensive care units, emergency rooms, delivery rooms, and special clinics. Prematurity is a risk factor that follows early labour, a planned caesarean section, or pre- eclampsia. Nursing and neonatal populations[edit]. A pediatric nurse checking recently born triplets in an incubator at ECWA Evangel Hospital, Jos, Nigeria. Healthcare institutions have varying entry- level requirements for neonatal nurses. Neonatal nurses are registered nurses (RNs), and therefore must have an Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree. Some countries or institutions may also require a midwifery qualification.[4] Some institutions may accept newly graduated RNs having passed the NCLEX exam; others may require additional experience working in adult- health or medical/surgical nursing.[5]Some countries offer postgraduate degrees in neonatal nursing, such as the Master of Science in Nursing (MSN) and various doctorates. A nurse practitioner may be required to hold a postgraduate degree.[4] The National Association of Neonatal Nurses recommends two years' experience working in a NICU before taking graduate classes.[5]As with any registered nurse, local licensing or certifying bodies as well as employers may set requirements for continuing education.[5]There are no mandated requirements to becoming an RN in an NICU, although neonatal nurses must have certification as a neonatal resuscitation provider. Some units prefer new graduates who do not have experience in other units, so they may be trained in the specialty exclusively, while others prefer nurses with more experience already under their belt. Intensive- care nurses undergo intensive didactic and clinical orientation in addition to their general nursing knowledge in order to provide highly specialized care for critical patients. Their competencies include the administration of high- risk medications, management of high- acuity patients requiring ventilator support, surgical care, resuscitation, advanced interventions such as extracorporeal membrane oxygenation or hypothermia therapy for neonatal encephalopathy procedures, as well as chronic- care management or lower acuity cares associated with premature infants such as feeding intolerance, phototherapy, or administering antibiotics. NICU RNs undergo annual skills tests and are subject to additional training to maintain contemporary practice. History[edit]The problem of premature and congenitally ill infants is not a new one. As early as the 1. It was not until 1. NICU).[9]Before the industrial revolution, premature and ill infants were born and cared for at home and either lived or died without medical intervention.[1. In the mid- nineteenth century, the infant incubator was first developed, based on the incubators used for chicken eggs.[1. Dr. Stephane Tarnier is generally considered to be the father of the incubator (or isolette as it is now known), having developed it to attempt to keep premature infants in a Paris maternity ward warm.[1. Other methods had been used before, but this was the first closed model; in addition, he helped convince other physicians that the treatment helped premature infants. France became a forerunner in assisting premature infants, in part due to its concerns about a falling birth rate.[1. After Tarnier retired, Dr. Pierre Budin, followed in his footsteps, noting the limitations of infants in incubators and the importance of breastmilk and the mother’s attachment to the child.[1. Budin is known as the father of modern perinatology, and his seminal work The Nursling (Le Nourisson in French) became the first major publication to deal with the care of the neonate.[1. Another factor that contributed to the development of modern neonatology was thanks to Dr. Martin Couney and his permanent installment of premature babies in incubators at Coney Island. A more controversial figure, he studied under Dr. Budin and brought attention to premature babies and their plight through his display of infants as sideshow attractions at Coney Island and the World’s Fair in New York and Chicago in 1. Infants had also previously been displayed in incubators at the 1. World Fairs.[1. 4]Early years[edit]. Bundesarchiv Bild 1. Berlin, Krankenhaus "Oskar- Ziethen", Kinderkrankenhaus. Doctors took an increasing role in childbirth from the eighteenth century onward. However, the care of newborn babies, sick or well, remained largely in the hands of mothers and midwives. Some baby incubators, similar to those used for hatching chicks, were devised in the late nineteenth century. In the United States, these were shown at commercial exhibitions, complete with babies inside, until 1. Dr A. Robert Bauer MD at Henry Ford Hospital in Detroit, MI, successfully combined oxygen, heat, humidity, ease of accessibility, and ease of nursing care in 1. It was not until after the Second World War that special- care baby units (SCBUs) were established in many hospitals. In Britain, early SCBUs opened in Birmingham and Bristol, the latter set up with only £1. At Southmead Hospital, Bristol, initial opposition from obstetricians lessened after quadruplets born there in 1. Incubators were expensive, so the whole room was often kept warm instead. Cross- infection between babies was greatly feared. Strict nursing routines involved staff wearing gowns and masks, constant hand- washing and minimal handling of babies. Parents were sometimes allowed to watch through the windows of the unit. Much was learned about feeding—frequent, tiny feeds seemed best—and breathing. Oxygen was given freely until the end of the 1. Monitoring conditions in the incubator, and the baby itself, was to become a major area of research. Most early units had little equipment, providing only oxygen and warmth, and relied on careful nursing and observation. In later years, further research allowed technology to play a larger role in the decline of infant mortality. The development of pulmonary surfactant, which facilitates the oxygenation and ventilation of underdeveloped lungs, has been the most important development in neonatology to date.[citation needed]Increasing technology[edit]. Neonatal intensive- care unit from 1. By the 1. 97. 0s, NICUs were an established part of hospitals in the developed world. In Britain, some early units ran community programmes, sending experienced nurses to help care for premature babies at home. But increasingly technological monitoring and therapy meant special care for babies became hospital- based. By the 1. 98. 0s, over 9. The emergency dash from home to the NICU with baby in a transport incubator had become a thing of the past, though transport incubators were still needed. Specialist equipment and expertise were not available at every hospital, and strong arguments were made for large, centralised NICUs. On the downside was the long travelling time for frail babies and for parents. A 1. 97. 9 study showed that 2. NICUs for up to a week were never visited by either parent. Centralised or not, by the 1. NICUs in saving babies. Around 8. 0% of babies born weighing less than 1. Review of Psychopathy. William D. Tillier; Calgary Alberta; Update: 2013-2017. There Are No Technology Shortcuts to Good Education « Educational Technology Debate. Kentaro Toyama. There are no technology shortcuts to good education. For primary and secondary schools that are underperforming or limited in resources, efforts to improve education should focus almost exclusively on better teachers and stronger administrations. Information technology, if used at all, should be targeted for certain, specific uses or limited to well- funded schools whose fundamentals are not in question. Caveat: Because this article was written for an audience most interested in government- funded primary and secondary education in developing countries, words like “wealthy,” “average,” and “typical” should be read with that context in mind. But, the conclusions are relevant for a broad class of primary and secondary schools in developed countries, as well.) To back these assertions, I’ll draw on four different lines of evidence. · 2. The history of electronic technologies in schools is fraught with failures. Computers are no exception, and rigorous studies show that it is incredibly difficult to have positive educational impact with computers. Technology at best only amplifies the pedagogical capacity of educational systems; it can make good schools better, but it makes bad schools worse. Technology has a huge opportunity cost in the form of more effective non- technology interventions. Many good school systems excel without much technology. The inescapable conclusion is that significant investments in computers, mobile phones, and other electronic gadgets in education are neither necessary nor warranted for most school systems. The mission of the Stanford Graduate School of Business is to create ideas that deepen and advance the understanding of management, and with these ideas, develop. In particular, the attempt to use technology to fix underperforming classrooms (or to replace non- existent ones) is futile. And, for all but wealthy, well- run schools, one- to- one computer programs cannot be recommended in good conscience. All of the evidence stands on its own, but I will tie them together with a single theory that explains why technology is unable to substitute for good teaching: Quality primary and secondary education is a multi- year commitment whose single bottleneck is the sustained motivation of the student to climb an intellectual Everest. Though children are naturally curious, they nevertheless require ongoing guidance and encouragement to persevere in the ascent. Caring supervision from human teachers, parents, and mentors is the only known way of generating motivation for the hours of a school day, to say nothing of eight to twelve school years. While computers appear to engage students (which is exactly their appeal), the engagement swings between uselessly fleeting at best and addictively distractive at worst. No technology today or in the foreseeable future can provide the tailored attention, encouragement, inspiration, or even the occasional scolding for students that dedicated adults can, and thus, attempts to use technology as a stand- in for capable instruction are bound to fail. With respect to sustaining directed motivation, even the much- maligned rote- focused drill- sergeant disciplinarian is superior to any electronic multimedia carnival. In an October 2. 00. ETD article, James Bon. Tempo also highlighted the importance of motivation. But, while Bon. Tempo suggested that we should seek technologies that motivate both teachers and students, I believe today’s technology is not up to the task. Note: The author retracts this statement and agrees with Bon. Tempo, as his articles actually suggest that even this is not possible if neither teachers nor students are motivated to begin with.])Subscribe now to follow this Educational Technology Debate via email updates sent to your inbox.. The Repetitive Cycle of Technology. For anyone concerned with high- tech in schools, two books are required reading as histories of technology and education. The first is Larry Cuban’s Teachers and Machines: The Classroom Use of Technology Since 1. American education up to the early 1. The second is Todd Oppenheimer’s The Flickering Mind: Saving Education from the False Promise of Technology. Oppenheimer also focuses primarily on US education, but updates and expands on Cuban’s findings for computers in schools through the early 2. Both authors consider the record of technology in schools and find it wanting. They reveal that while technologies can have positive educational impact in restricted instances, successes pale in comparison to failures overall. By not knowing this past history, we seem condemned to repeat it over and over and over. One point that both authors make is that there is a repetitive cycle of technology in education that goes through hype, investment, poor integration, and lack of educational outcomes. The cycle keeps spinning only because each new technology reinitiates the cycle. In 1. 92. 2, Thomas Edison claimed that movies would “revolutionize our educational system.” In 1. William Levenson, a Cleveland radio station director, suggested that portable radios in classrooms should be “integrated into school life” alongside blackboards. In the 1. 96. 0s, governments under John F. Kennedy and Lyndon Johnson invested in classroom TV. In an irrational leap of reasoning that is symptomatic of technology in education, Johnson went from a valid lament, “Unhappily, the world has only a fraction of the teachers it needs,” to a non- solution… to meet the challenge “through educational television.” The hubris and failures of technology projects are detailed by Cuban and Oppenheimer, but with hindsight available to all of us, we know that none of these technologies has delivered on their promises. If anything, we have become wary of their educational power. For example, on the one hand, television excels as a medium for delivering information. Seduced by this capacity in 1. Wilbur Schramm, the father of communications studies, asked “What if the full power and vividness of television teaching were to be used to help the schools develop a country’s new educational pattern?” He was thinking, in particular, of mass media’s potential to transform education for developing countries. The transformation never occurred, probably because as motivational as television can be, it still falls far short of generating the motivation required for education. For every person who falls prey to Madison Avenue’s latest advertisement, hundreds of others just ignore it or turn the channel – if that’s true of the most persuasive television commercials, why should we expect television to be able to regularly sustain the motivation (and not just the attention) of easily distracted children to do the cognitive push- ups that education demands? In the meanwhile, many of us have come to sense television’s shortcomings. Educated parents restrict their children’s time in front of the TV, and many households ban television altogether – at its best, television is considered a cheap babysitter to hold a child’s attention when adult attention is scarce; at its worst, television caters to our weakest impulses, glamorizes materialism, desensitizes us to violence, and lulls us into a zombie- like trance. As a result, most people today would laugh at a school system based on watching broadcast television programs, however educational. Yet, that was exactly the idea behind an experiment in American Samoa in the mid- 1. The program was dismantled several years later as teachers, administrators, parents, and even students expressed dissatisfaction with the students’ academic performance. Computers: The Latest Technology Cycle. Today, computers and mobile phones are the shiny new technologies, and they offer an even more seductive promise. One argument goes that it was the passiveness of older technologies that was the problem, so today’s interactive digital technologies are the perfect solution. A systematic review of current and emergent manipulator control approaches. Pressing demands of productivity and accuracy in today’s robotic applications have highlighted an urge to replace classical control strategies with their modern control counterparts. This recent trend is further justified by the fact that the robotic manipulators have complex nonlinear dynamic structure with uncertain parameters. Highlighting the authors’ research achievements in the domain of manipulator design and control, this paper presents a systematic and comprehensive review of the state- of- the- art control techniques that find enormous potential in controlling manipulators to execute cuttingedge applications. In particular, three kinds of strategies, i. PID) scheme, robust control and adaptation based approaches, are reviewed. Future trend in the subject area is commented. Lord Commander Militant of the Astra Militarum. Military Forces. Astra Militarum (Imperial Guard) regiments. Establishment. Imperial Reformation (3. Millennium)"Men, we are the first, last and often only line of defence the Imperium has against what is out there. You and that fine piece of Imperial weaponry you hold in your hands is all that is keeping humanity alive. Most of you will probably not live to see your second year in the Guard and most of you will probably never see your homeworlds again, but I can guarantee you that when you do fall, with a prayer to the most high and mighty God- Emperor on your lips, you will have earned the right to call yourself a man!"— Staff Sergeant Vermak, 1. Cadian Shock Regiment. The Imperial Guard in action. The Astra Militarum, also known as the Imperial Guard in colloquial Low Gothic, is the largest coherent fighting force in the galaxy. They serve as the Imperium of Man's primary combat force and first line of defence from the myriad threats which threaten the existence of the human race in the late 4. Millennium. It is comprised of countless billions of men and women - - hundreds of thousands of different regiments, supported by a vast array of light and heavy armoured vehicles that provide the Guard's primary offensive punch. They are usually the first Imperial force to respond to a threat if a planet's Planetary Defence Force (PDF) fails to suppress it. They also garrison major locations of strategic or cultural interest to the Imperium and are often found in defensive roles. The Imperial Guard is often referred to as the "Sledgehammer of the Emperor"; the sheer amount of force that the Imperial Guard can bring to bear on the enemy is devastating, but is not as direct or as precise as their Space Marine allies, who are described as the "Scalpel of the Emperor." There is no universal uniform or regimental command hierarchy in the Imperial Guard, although it is compulsory for every regiment to have at least one Commissar to maintain the discipline and morale of the men while watching for any signs of corruption or taint in the ranks. However, it is worth noting that a number of worlds copy the uniform used by the regiments of the Cadian Shock Troops, who are viewed as being the most effective of the Imperial Guard's infantry units. The main tactic of the Imperial Guard is to overwhelm the enemy with their endless numbers, while at the same time hammer them into submission with devastating artillery and crush them with powerful main battle tanks. History"When the people forget their duty they are no longer human and become something less than beasts. They have no place in the bosom of humanity, nor in the heart of the Emperor. Let them die and be forgotten."— Prime Edicts of the Holy Synod of the Adeptus Ministorum. The forces of the Armageddon Steel Legion rally on Cadia during the Battle of Tyrok Fields. The Imperium is vast on a scale incomprehensible to mortal man. Communication and travel between the glinting motes of this far- flung stellar empire are laden with risk. The inimical realm of the Warp provides the only conduit for interstellar movement or messages yet it also taints or temporally displaces much of that which plunges into its depths, confounding the Imperium's attempts at centralised control or unified strategy. We would like to show you a description here but the site won’t allow us.On every border and every. In these dark times, warfare on a galactic scale is a matter of soulless, grinding logistics. Only the Astra Militarum can marshal the manpower to fight such a war. The Imperial Guard fights punishing battles of attrition in which incalculable lives may be expended for each objective achieved. An Imperial Guard army must utilise the twin advantages of vast numbers and overwhelming firepower to annihilate its foes. Where xenos aircraft dance and weave with impossible grace, the Imperial Guard simply fills the sky with a thunderstorm of munitions from which no amount of aerobatic skill can save the foe. Where heretical bastions stand defiant, Imperial Guard commanders call down artillery bombardments that reduce all to rubble with their apocalyptic fury. The greatest enemy threats are torn apart in the crossfire of thousands of heavy weapons, or smashed aside by the gallant charge of hundreds of Imperial tanks. The enemies of Mankind may employ dark sciences or alien weapons beyond humanity's ken, but such deviance comes to naught in the face of honest human intolerance backed by a sufficient number of guns. For all the might of its armour and artillery, the true backbone of the Astra Militarum is the countless waves of infantry who take to the field. The sheer scale of the battles fought by the Imperial Guard is dehumanising in the extreme. Entire regiments of brave warriors are reduced to statistics upon the scrolling screens of Imperial strategoes, grains of sand sliding through the fingers of greater and more privileged individuals. Yet every single company, every single squad, every single Imperial Guardsman who lifts their Lasgun and takes a stand in defence of their species, is crucial. Without a constant deluge of new recruits, the Imperial Guard would cease to function. A battle may see the deployment of millions, yet time and again it is a single heroic company who carry their charge to secure a crucial gatehouse or pivotal objective. Squads of desperate men battle impossible odds, with nothing but their courage and faith driving them to hold the line while their valuable betters are evacuated to safety. Every day that the Imperium endures, Imperial Guardsmen stand in the face of beasts more hellish than their worst nightmares. Men charge screaming alongside their comrades into the mouth of hell, Lasguns spitting death at the foe even as xenos munitions tear bloodied holes in the human ranks. In a galaxy of never- ending warfare, what makes the perpetually outmatched men of the Imperial Guard so admirable is that they know near constant fear, yet they lift their weapons, plant their feet, and fight on regardless. Their lives may be short and brutal, their sacrifices insultingly thankless, but it is because of the Imperial Guard that the Imperium continues to weather the storm of these dark times. Wars of Unification. The Urslavik 1. 2th Infantry, one of the genetically- engineered Genos regiments employed by the Emperor of Mankind during the Unification Wars on Terra. During the turbulent era known as the Age of Strife, the Sol System and the nearby star systems that had been colonised by humanity during the Dark Age of Technology were effectively cut off from interstellar travel or communication with each other due to the massive Warp Storms that swept the galaxy. Little remained of the once sophisticated civilisation of Old Earth's glorious past as the centre of a growing human interstellar civilisation marked by advanced science, high culture and wondrous technologies. Techno- barbarian warlords and their warrior hordes continuously fought over the planet, which had become little more than a massive battleground for their wars of attrition. They made use of chemical, biological and even thermonuclear weapons of mass destruction, and slowly transformed the cradle of Mankind into a battered, post- apocalyptic wasteland across most of its scarred surface. It was against this backdrop of oppression, violence and casual brutality that the Emperor of Mankind first revealed Himself to the people of Terra. In secret, He had been planning for this moment in history for millennia, ever since the Age of Strife had fractured what remained of the ancient human federation which had once stretched across a part of the galaxy. The Emperor moved to create the military organisations he would need to begin the reunification of Mankind, and He used the raw materials at hand after millennia of savage conflict between the techno- barbarian nation- states of Old Earth. |
